Improving Lives With Data

ILWD is a place-based analytical and program design initiative focused on identifying and addressing unmet need among high-risk older adult populations who are often not captured through conventional service-intake pathways. This repository contains the analytical infrastructure, documentation, and supporting materials developed to guide community-level planning and institutional adoption.

Purpose

ILWD was developed to:

  • Identify high-risk older adult populations with limited visibility in existing systems
